2010-2011 BETHLEHEM BLAST Coaching Evaluation Form

     Please fill in the form below. Once completed to your satisfaction click "Submit".  We are interested in your evaluation of the 2010-11 coaches for your son's/daughter's team. If you prefer, you can print the blank form, fill it in manually, place in an envelope addressed to Bob Fyrer, Head of Coahces and hand deliver it to him, or put it in the BLAST mailbox at the Steel Ice Center.

    This information will help us ensure that our coaches are continually improving and will be valuable in helping us determine our coaching selections for the next season.The information you provide will be held in strict confidence. You can use this form at any time during the season to provide us with feedback. Please refrain from personal attacks and referencing isolated situations. The Hockey Operations Group is interested in constructive feedback and determining the direction that coaching should take in the future in the BETHLEHEM BLAST Youth Hockey Organization.
