2010-2011 Bethlehem Blast Registration
Registration fee is non-refundable and is credited towards your season fees once you are placed on a BLAST roster.

BETHLEHEM BLAST Forms:

All of the BETHLEHEM BLAST forms are viewable with Adobe Acrobat. Adobe Acrobat is a free software available for download.

PLAYERS:

To register you must complete the following forms and deliver them to the BLAST with your non-refundable registration fee:
1. BLAST Player Registration Form
2. DVHL Player/Parent Agreement (Code of Conduct) Form
3. USA Hockey Consent to Treat Form & Medical Release Form
4. USA Hockey Waiver of Liability Form
5. St. Barnabas Online Sportsmanship Course (instructions found with the         BLAST registration form

6. Jersey Number Request Form (New Players Only)
7. USA Hockey Youth Transfer Form (Non-U.S. Citizens Only)

All forms are linked below.  The Registration Form includes a checklist to help insure that you have all the proper paperwork to complete the process.

All players will be required to register themselves with USA Hockey for the 2010-11 season.  Fee is $40 and is not included in BLAST season fees.  The BLAST will not be registering players with USA Hockey.  All players will need to provide a copy of their online USA Hockey registration form (with bar code) before the start of the 2009-10 season.  This change is at the request of the DVHL, the Atlantic District, and USA Hockey.

All new players wishing to register with the BLAST who are not currently registered with USA Hockey will need to obtain such after April 1, 2010 and provide a copy of the confirmation with bar code before they can participate.

Iif you are a player who was a member of another DVHL program in 2009-2010.  We cannot accept your application prior to April 20th at 5:00 PM.
Unless you are registering for our Girls Program then it is April 19th.  Please see cover sheet for details.  This is a correction from prior date of 4/12.
